TYPE OF INFORMATION COLLECTED

In the course of operating the Website we will collect (and/or receive) the following types of information when you sign up to our Service and/or engage in transactions via our Website that can be used to identify you. You authorize us to collect and/or receive such information.

Personal Information: first name and last name, mailing address, shipping address, telephone numbers, e-mail address, PayPal address (if different from e-mail address), date of birth, gender, age, username, password and other personally identifiable information.
If you post reviews, comments, images or other content on our Website, anything that you post becomes public to all Website users – whether it is personally identifying of you or not — and we disclaim any responsibility for any subsequent use by anyone else of any such information that you post publicly. Wellness Information: short-term and long-term health goals, your diet, your lifestyle, your views about health/wellness, and wellness issues that you have experienced or that exist in your family if needed. However, we do not collect any information that would be considered “protected health information” under the Health Insurance Portability and Accountability Act of 1996.
Billing Information: cardholder transaction data, card numbers, expiration dates, billing addresses and CVV codes. Such Billing Information will be collected and processed by our Third Party Payment Vendor pursuant to the terms and conditions of their privacy policies and terms of use, and we do not obtain access to any Billing Information.

Order Information: the Products you purchased from your account or would like to purchase.

Other Information: Such Other Information may include:

From Your Activity: IP addresses and other technical information collected from the servers utilized by you or your vendors, browser type and language, referring and exit pages and URLs, date and time, amount of time spent on particular pages, what sections of the Website Visitors visit, service provider name and the date, time and content of any messages sent from you to us, and similar information concerning your use of the Services.

Our payment processor stores all credit-card information; mendafter.com itself does not retain credit card numbers.
